Studies on 26 white male Wistar rats have demonstrated that the development of experimental pancreatitis activates proteases, locally in the peritoneal secret, systematically - at the blood level, and distantly - at the lung level. The activation of is the most pronounced, elastase at both the local and systemic levels. This may be indicative of the important role of elastase activation in the pathogenesis of pancreatitis, in which, at the local level, the elastase can be a leading enzyme that leads to destruction of the pancreas. A slight increase in the antiprotease activity in the peritoneal secret and its reduction in the blood indicate that the inhibitors control the increase in the activity of proteases inadequately. An adequate raise of the antiprotease activity is observed only in the bronchoalveolar secret, which apparently is of a compensatory character and appears to be a factor protecting the lungs from systemic proteolytic aggression.

In a pilot study on white laboratory rats at the age of 1.5 months (early puberty) with simulated adjuvant arthritis we investigated the morphological changes of the parotid gland. Early signs of changes that are specific to the development of autoimmune inflammation, as well as warning signs of decline in the functional activity of the organ were established. The absence of gross sclerotic changes at an early stage of development of the autoimmune reactions of the salivary gland provides a basis for further research to develop therapeutic and preventive measures.

The article presents a study of diagnosis and correction of dry eye syndrome among users of soft contact lenses as function of the age and type of the material of soft contact lenses. We examined 136 patients (272 eyes) aged 10 to 35 years. The users of soft contact lenses were divided into three groups; the first group use hydrogel SCL for vision correction, the second group use silicone-hydrogel SCL, the third group are the primary patients for whom SCL are to be selected. The survey was carried out before treatment, after one month and after six months of treatment. It has been established that the majority of users, in presence of risk factors, require maintenance therapy with non-conservation drugs while wearing SCL.

We have revealed elevated levels of CD4 + T-cells, combined with a reduction in CD8 + -cells in the serum in COPD patients with reduced content of 25(OH)D3 , which creates conditions for development of autoimmune reactions in COPD patients with vitamin D deficiency. We consider these facts as a pathophysiological basis to recommend vitamin D for correction of the immune status in patients with stage I-III COPD in the remission phase on the background of vitamin D deficiency.

The research of the bony tissue structure, specifically, its functional state in children has been carried using the ultrasound densitometer. We have revealed solidity deterioration in 9.38% of children. It is reflected in the osteopenia state of the patients. The revealed interactions between the densitometer data and adipokins are suggestive of a protective leptin effect on the trabeculate bone tissue and a negative adiponektin effect on the osteogenesis in children.

Dystrophic disorders accompanied with cardiovascular dysfunction, reducing of functional reserves of oxygen-related bioacidity of neutrophils, decreasing of the glutathione level, and appearance of endotoxemia have been revealed through investigation of functioning of both the oxidant-antioxidant and cardiovascular systems. Decreasing of the heart rate at rest, isopropylnoradrenaline-induced myocardial ischemia, cardiac arrhythmia, reducing of chronotropic response have been found in the experimental animals as compared to the intact ones.

We have clinically investigated the condition of the parodont and performed osteodensitometry of the calcaneal bone tissue, rheoparodontography, measured the level of leptin, immunoglobulins IgA, IgM, IgG, interleukin SILT-1ß, IL-4, IL-6, FNO-α, secretory immunoglobulin A in patients with abdominal obesity. This disease is associated with formation of a generalized periodontal disease of a chronic course. Inflammatory changes in the periodontal tissues are accompanied by a significant increase of the levels of IL-6, FNO-α, synthesized by adipocytes in the oral liquid and blood serum, a decrease of the mineral density of the bone tissue, an increase of the peripheral vascular resistance, and a reduction of the microcirculatory blood supply for the parodont tissues. Patients with obesity are typically featured with development of hyperleptinemia. We have established direct mutual correlation between the leptin level in the blood serum and clinical laboratory indicators of inflammation activity in the parodont.

The variation of the parameters of the emissary veins has been investigated as function of the fetal development term. The research was performed on 70 fetuses and newborns of both sexes for fetal development terms of 20 to 40 weeks. Corrosion preparations of the head venous system were made, then, morphometry of the obtained preparations was performed. We have found that in the prenatal stage of development the emissary veins are well-developed and their sizes are large enough. All the corrosion preparations have emissary vein, the largest and most persistent ones being mastoid and condylar emissary veins.

Combined examination and surgical treatment of 39 patients with chronic odontogenic maxillary sinusitis have been performed. The study group included 12 patients who underwent extranasal maxillary sinusotomy according to the method we developed, with repair of the trepanation defect of the maxillary sinus front wall with a triangle-shaped auto-transplant (rotated replant). The control group (27 patients) underwent total extranasal maxillary sinusotomy in Zaslavsky-Neuman modification according to the generally accepted method, with closing of the trepanation defect of the operated sinus front wall by the mucous-periosteal flap. Positive early and long-term results of the surgical treatment in patients with the chronic odontogenic maxillary sinusitis with plastic closure of the defect of the maxillary sinus front wall by polygonal rotated bone replant are evidence of the efficiency of the proposed method.

The dynamics of the serum oxyproline level in patients with myocardial infarction on the background of undifferentiated connective tissue dysplasia have been studied. It was found that the increase of serum oxyproline on the 1st day of myocardial infarction occurs in patients both with and without undifferentiated connective tissue dysplasia. In patients with connective tissue dysplasia the level of serum oxyproline was growing up to the 28th day after the infarction development. These patients were also characterized by a more expressed post-infarction cardiac remodeling, namely, dilatation of the left atrium and left ventricle and reduction of the left ventricular contractility, unlike what was observed in the patients without connective tissue dysplasia.

We have formed 2 groups, each of them included 62 patients; the first (control) group was taking the standard conservative therapy of the retinal vein thrombosis; the second (comparative) group was taking, along with the standard therapy, Aflibercept, intravitreally, 2 mg in 0.05 ml. In patients with retinal vein thrombosis that entered the in-patient hospital department during the first week, the TGFα level did not change; while the TGFβ level increased 10 times and more. However, the number of patients at the early stage was not large enough for us to be sure about the data reliability. In the development of post-thrombotic neovascular glaucoma, the TGFα concentration increased 1.7 times and TGFβ increased 28 times in the lacrimal liquid. TGFβ is one of the key mediators of the fibrovascular scar formation at the end of subretinal neoangiogenesis, and its increase is a prognostically unfavorable factor; its dynamics is an indicator of the treatment effectiveness. Application of the angiogenic therapy, especially agents of the VEGF-Trap group, allowed for a significant decrease of the TGF α and TGF-β concentration in the lacrimal liquid. According to the data of optical coherent tomography, a renewal of the relief and foveolar cavity takes place; as well as nearly a double reduction of the edema and a change of the edema character.

The purpose of the research is to optimize IAP monitoring in patients at the postoperative period. We have compared the results of the measurement of the intra-abdominal pressure in 54 patients at the postoperative period through the bladder and by the apparatus of our design. The comparison has shown that the data obtained by our apparatus give more accurate results, and the method would be more accurate and safe in clinical practice for monitoring pressure in the abdominal cavity in patients at the early postoperative period. This method can be used to control intra-abdominal pressure during the postoperative period at any hospital.

The article is devoted to solving the actual scientific problem of experimentally basing the effectiveness of isogenic cell cardiomyoplasty in myocardial infarction. The use of embryonic, mesenchymal and committed mesenchymal stem cells (MSC) and introduction of the granulocyte colony-stimulating factor have been comparatively analyzed. It has been shown that the most promising is the application of MSCs. In has been proved that the less is the period since the moment of the simulation, the more effective is the transplant performance on the remodeling of the heart. Autologous MSCs used in acute myocardial damage act as inducers of the regeneration processes for remodeling the damaged myocardium, providing reparative morphogenesis and an increase of the adaptive resources of the preserved, i.e. viable myocardium.

The morphometric parameters of the pterygoid venous plexus (length, width, height) have been studied on 40 corrosive preparations of fetuses with gestation terms of 20 to 32 weeks. The preparations were divided into three groups according to the shape of the head (brachycephalic type of structure, mesocephalic, dolichocephalic). All the types of sizes of the plexus were predominant in the mesocephalic, while in the dolichocephalic group the sizes were minimal. In all the three groups, a significant predominance of the anteroposterior size of the plexus was observed on the right side.

We have studied the effect of C3 -to-Th8 traction muscle relaxation in the areas of vascular reflex zones on oxygenation of intact tissues of the extremities in athletes with predominantly aerobic training process (soccer player and athletes, 30 persons) and anaerobic training process (wrestlers and boxers, 30 persons. It has been established that C3 -to-Th8 traction muscle relaxation causes a significant increase in tissue oxygen saturation of the forearm and shank in accordance with the type of the muscular hypertrophy due to various specializations of athletes.

The study has revealed a positive dynamics of the clinical course and immunological parameters of the peripheral blood in patients with plaque psoriasis under the influence of peloids of the balneotherapy resort Saki Lake. In patients treated with therapeutic mud and brine baths in combination with systemic use of alpha-lipoic acid «Berlition», the dynamics of clinical and immunological indicators, taking into account the blood cytokine profile, was stronger; “clinical recovery” was achieved in 87,5 % of cases.

The article presents an analysis of the dynamics of morbidity and mortality indicators for cardiovascular diseases of the male population in the Republic of Crimea. The study has revealed that in the structure of general morbidity of the Republic of Crimea the cardiovascular diseases are the first on the list and constitute 45.1% of all the pathologies, whereas the incidence in the primary structure of these diseases takes the third place (14.7%). The overall incidence of male CVD increased from 2008 to 2011 by 13.4%, and in 2012 it decreased by 0.8%. There was an increase in the incidence in the primary structure till 2009 up to 3%; then, a decline by 17.6% was observed. In the Republic of Crimea, cardiovascular diseases are the main cause of death and constitute 67.4% of all the cases, with the share of coronary heart disease accounting for 77%, stroke accounts for 8.7%. In 2013, there was an increase in mortality from cardiovascular diseases among both sexes: from 914.2 persons per 100.000 in 2012 up to 917.6 persons per 100.000 in 2013 (among the male population, the figure was 871.3 per 100.000).

In this experimental research we studied the stages when occurs change of regenerative wound healing process in burn wound and coupled with a combination of several affecting factors. The experiment included 16 whites adult male Vistar line rats, weighing 250-300 g, of the same size and age. Animals were divided into 2 groups. In the I group included animals which burns combined with a fracture of the femur (8 rats) - the main group; Group II (control) - rats, only with thermal injury (8 rats). In comparing of the macroscopic data by everyday observations of the wound and morphological signs of wound healing process in burn wounds, we detected: in the control plots of burn wounds were occurred rejection of necrotic crust and formation of granulation tissue at 7,25 ± 0,37 day, whereas, in the observation group similar changes were noted only at 11,75 ± 0,25 day, which was reflected in slowing down of proliferation rate of vascular netting and the development of granulation tissue.

The occurrence of purulent septic complications in the operative gynecology is 5% to 20%. One reason for the complications is the use of sutures that are essentially foreign bodies and are accompanied by a pronounced reaction of the surrounding tissues in the area of the alloying tissues. To solve the problem, Russian specialists created a medical welding system that minimizes the use of suture material. We have established that low sensitivity of the thrombocyte receptors to FAT on the seventh postoperative day in the patients of group II is a manifestation of a stronger inflammatory reaction at the suture site than what is observed with the biological tissue welding. The use of the welding station in operative gynecology reduces the operation duration by 7 minutes; moreover, there is a more favorable course of the reparative processes and a twofold decrease of the suture material expense. In addition, the occurrence of purulent septic complications is reduced, which allows us to confine ourselves with mono-antibacterial therapy at the postoperative period.

ВЗАИМОСВЯЗЬ КЛЕТОЧНОГО И ГУМОРАЛЬНОГО ОТВЕТА ИММУНИТЕТА НА ЭНДОТОКСИН С УРОВНЕМ ЛЕЙКОЦИТОВ И СРБ У БОЛЬНЫХ СИСТЕМНОЙ КРАСНОЙ ВОЛЧАНКОЙ

Systemic lupus erythematosus (SLE) is one of the most serious autoimmune inflammatory connective tissue diseases with unknown etiology. Among the possible trigger factors can be a self-sustaining inflammation endotoxin of gram-negative enteric bacteria. In this article, we studied the immune specific response to endotoxin, as well as white blood cell count and CRP in SLE patients compared with the control group. The study group included 48 patients with SLE I and II activity degree. The control group consisted of 40 apparently healthy donor. The cellular element of immune response to endotoxin was studied by determining the endotoxin-binding receptors on granulocytes and monocytes by flow laser cytometry. Antiendotoxin concentration of antibodies and CRP were determined by ELISA. The level of white blood cells and its populations were determined by microscopic method with color smears by Romanovsky-Giemsa. The results were processed statistically using licensed software. In the study SLE patients showed an increase in the concentration of antiendotoxin antibodies, CRP, and decreased expression of endotoxin binding receptors on monocytes and granulocytes. Also the correlation relations combining cellular and humoral immune response to the endotoxin with the degree of disease activity, CRP and white blood cells, band neutrophils were found. These results confirm the existence of an imbalance in the immune system during the response to endotoxin, as well as its participation in the selfsustaining autoimmune inflammation in SLE patients.

We have investigated the impact of electromagnetic radiation (EMR) of mobile phones (MP) and personal computers (PC) in 60 white Wistar rats, divided into 3 experimental groups (Group 1 - MP, Group 2 - PC, Group 3 - PC + MP) and the control group (C). The experiment lasted for 26 weeks. The weight was measured weekly with calculation of its relative growth. At the end of the experiment, after giving ether anesthesia, the animals were decapitated. We isolated and weighed the spleen, liver, and both testicles. The relative mass (RM), i.e. organ mass/whole body mass ratio was calculated. We extracted the bone marrow of the femur to obtain impression smears and obtained smears from the spleen as well. By the end of the experiment, as compared to the control group, the relative body mass values of the laboratory animals were as follows: Group MP - 1.039 ± 0.095 (p = 0.992), Group PC - 0.945 ± 0.111 (p = 0.443); MP + PC - 0.631 ± 0.114 (p = 0.032). Determining the relative mass of organs, we revealed significant differences in the RM values of Group MP for the liver, spleen and testes (p = 0.045; p = 0.016 and p = 0.016 respectively). In order to achieve a successful administration of genetically engineered biological drug therapy in patients with rheumatoid arthritis it is essential to conduct examinations aimed at detection, diagnosis and prevention of tuberculosis in this group of patients. In view of wide administration of genetically engineered biological products and significant increase of the number of patients taking them for a long time, it should be reasonable to consider a new group of patients with a high risk of tuberculosis infection. All the patients who are administered to have genetically engineered biological drug therapy need to undergo a comprehensive examination in order to exclude development of active TB and monitor latent tuberculosis infections
